Social Prescribing in East Riding
Hi everyone. My name is Charlotte and I am a Primary Care Network CommunityLink worker in Pocklington and Stamford Bridge.
We all have issues in our lives which may affect our health and wellbeing in some way. The pandemic has also set us through some challenging times, and you may have faced a bereavement, struggles with your emotional wellbeing, isolation, finances, employment and more. The good news is that there is support available and we are here to help you overcome these challenges you may be facing.
As CommunityLink workers we are here to offer social prescribing support to adults across East Riding. Social Prescribing can help with personalised solutions to social, emotional or practical needs, leading to improvements in your health and wellbeing. Sometimes it can be as simple as getting you connected to a local group or support service, and we can signpost you to the right support out there. We can also provide you with some self help and care guidance as well as listening and encouraging you along the way.
We work closely with our other YourHealth services, including the Healthtrainers and Mental Health and Wellbeing Coaches, to ensure we are looking at ALL aspects of your wellbeing. This service is completely free and available at your GP practice in East Riding.
